What You'll Do:

The LPN for Assisted Living plays a key role in providing high-quality resident care, medication administration, and clinical oversight in accordance with physician orders, facility policies, and state regulations. Administer medications and treatments, ensuring accuracy and compliance with physician orders and nursing standards Monitor resident health status, recognize changes in condition, and promptly report concerns to physicians and appropriate staff Document assessments, observations, and nursing care in the electronic medical record (EMR), including MAR/TAR entries and nursing notes Support resident care planning by participating in assessments, reassessments, and service plan updates with the interdisciplinary team and families Supervise and support Certified Nursing Assistants (CNAs) and Medication Aides while ensuring quality care delivery Ensure safe medication management, including narcotic counts, shift-to-shift accountability, and compliance with facility protocols What We're Looking for in an
