The ideal candidate will provide exceptional, patient-centered care in a supportive and professional environment.

Key Responsibilities Patient Care:

Administer medications, tube feedings, and treatments as prescribed by physicians. Monitor and record vital signs while promptly reporting any changes in patient condition.

Documentation:

Maintain accurate, detailed, and compliant patient records in accordance with legal, ethical, and agency standards.

Collaboration:

Work closely with Registered Nurses, physicians, and other healthcare professionals to implement individualized care plans.

Patient & Family Education:

Provide education on medical conditions, treatment plans, and preventive health measures.

Emergency Response:

Respond effectively to medical emergencies, providing immediate and appropriate interventions.

Supervision:

Support and guide nursing assistants and other care staff as needed to ensure high-quality service delivery.

Qualifications Education:

Graduate of an accredited nursing program.

Licensure:

Current and valid New Jersey LPN license (required).

Experience:

Previous nursing experience in home care, long-term care, or clinical settings preferred.

Skills:

Strong clinical judgment, attention to detail, and ability to work independently or as part of a team.

Certifications:

Current BLS (Basic Life Support) required; ACLS (Advanced Cardiovascular Life Support) preferred.

Communication:

Excellent verbal and written communication skills; compassionate and professional bedside manner.
