Our openings include full-time, Night Shift! Day in the Life / Expectations Provide patient care to residents Comply with state and federal regulation Work collaboratively with members of the healthcare team. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Assess and coordinate patient's discharge planning needs with members of the healthcare team.

Requirements:

Valid state license(s) Excellent verbal communication skills and strong interpersonal skills High integrity and well-grounded work ethic Agility and flexibility with changing priorities Team-oriented and a practical team-player Professional demeanor At least one year experience providing care and services in long-term care (preferred) We look forward to welcoming you to our team!
