Job Summary and Qualifications Summary:

The Licensed Practical Nurse (LPN) within the PACE program plays a vital role in supporting our participants through comprehensive medication oversight and team coordination. Guided by the PACE model of care, which emphasizes an interdisciplinary, participant-centered approach focused on promoting independence and allowing individuals to remain safely in their homes and communities, the LPN serves as an essential member of the clinical care team. This position is responsible for ensuring medications are administered and documented accurately, assessing changes in condition, and providing hands on care. In collaboration with providers, pharmacy, caregivers, and the interdisciplinary team, the LPN supports communication, care coordination, and continuity of services to promote optimal participant outcomes and enhance quality of life. Provides direct patient care in an outpatient setting, evaluates outcomes and adjusts nursing care processes indicated to adhere to legal, ethical, and clinical standards.

Performance Criteria:

1. Delivers quality customer service to all patients and family members. Makes clinical judgement based on their symptoms to determine if the need to see the physician is necessary. Coordinates care as indicated with specialist providers and nursing facilities 8. Coordinating with care team to implement goals and interventions as it relates to the clinical team 9. Assessing participants medical needs and interventions to create success in the community and creating plan of care to support needs 10. Coordinator of care, orders, and interventions as prescribed with SNF Partners.

Performance Criteria:

1. Regularly documents and reports data, patient response and progress. Individualizes patient teaching and care plan based on patient's needs. Helps to achieve desired outcomes by serving as an advocate for patients and families. Identifies problems and implements measures to meet needs of patients and families throughout continuum of care.
