Full Job Description Salary:

$35 / per hour

Year of Nursing Experience Required - Tracheostomy and Ventilator experience preferred Maxim Healthcare is hiring for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to work on our Transition team, for full-time night shifts. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim. Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-90 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then phased out of the home.

Responsibilities:

Utilize the nursing process to assess, plan, implement, and evaluate patient care. On each assignment, provides skilled nursing care/services in accordance with prescribed orders Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition. Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

Currently licensed as an LPN/LVN in the state in which the LPN/LVN will practice. Current TB or Chest X-Ray. Current BLS card.
