Job description: LPN/RN Baylor provides skilled nursing care to residents in a nursing home or long-term care facility. The LPN/RN is responsible for assessing resident health needs, developing and implementing care plans, administering medications and treatments, supervising nursing staff, and ensuring compliance with regulatory standards. The Baylor LPN/RN promotes resident-centered care, safety, and quality of life. Essential Duties and Responsibilities Assess residents' physical, emotional, and psychosocial needs on admission and on an ongoing basis Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team Administer medications, treatments, and IV therapy as prescribed Monitor residents for changes in condition and respond appropriately to emergencies Document nursing care accurately and timely in accordance with facility policies and regulatory requirements Supervise, mentor, and delegate duties to Certified Nursing Assistants (CNAs) Ensure compliance with federal, state, and local regulations, including infection control and safety standards Communicate effectively with residents, families, physicians, and healthcare team members Participate in care conferences, quality assurance activities, and in-service education Provide wound care, pain management, and end-of-life care as needed Advocate for resident rights, dignity, and quality of life Qualifications Current, active LPN/RN license in the state of employment Graduate of an accredited nursing program (Associate or Bachelor's degree in Nursing) Minimum of 1 year of nursing experience preferred; long-term care or geriatric experience strongly preferred Current CPR/BLS certification Knowledge, Skills, and Abilities Strong clinical assessment and critical-thinking skills Knowledge of long-term care regulations and standards of practice Ability to supervise and work effectively with nursing staff Excellent communication and interpersonal skills Proficiency with electronic health records (EHR) Ability to manage multiple tasks in a fast-paced environment Compassionate, patient-focused approach to care Physical Requirements Ability to stand, walk, bend, and lift residents or equipment as required Ability to assist with resident transfers and mobility Visual and hearing acuity sufficient for patient assessment and documentation Work Environment Nursing home or long-term care facility May involve exposure to infectious diseases, bodily fluids, and emergency situations Flexible scheduling, including nights, weekends, and holidays Equal Opportunity Statement McCoy Memorial Nursing Home is an equal opportunity employer and does not discriminate on the basis of race, color, religion, sex, sexual orientation, gender identity, national origin, age, disability, or veteran status.
