• Valid LVN license
• Proficient in EHR systems
• Strong patient care and monitoring skills
• Knowledge of medical terminology
• Experience in discharge planning
• Excellent communication skills
• Ability to work in home health setting
• Familiarity with catheterization and tube feeding
• Basic life support certification from the American Heart Association or American Red Cross.
• Understanding of HIPAA regulations Responsibilities
• Provide direct patient care in a homecare setting, focusing on individualized treatment plans and compassionate support.
• Conduct patient assessments, including catheterization, tube feeding, and monitoring vital signs.
