Job Description

Home Health LPN Guthrie, OK (and surrounding areas) Full-Time | Company Car Provided. Are you a compassionate LPN who wants to deliver real, one-on-one care instead of rushing between hospital rooms? Join Companion Healthcare as a Home Health LPN and help patients receive skilled nursing care in the comfort of their own homes — where they feel safest and most independent..In this role, you'll build meaningful relationships with patients and families while making a genuine difference in their quality of life every single day. Best of all? We provide a company car so you can focus on care instead of worrying about mileage or vehicle wear. What You'll Do Perform comprehensive assessments and document patient condition at every visit Monitor vital signs, review medications, and complete evaluations Administer physician-ordered medications safely and accurately Provide skilled wound care, dressing changes, and track healing progress Educate patients and families on home care techniques, medication management, and health strategies Collaborate with the care team to deliver personalized, high-quality support What We're Looking For Graduate of an accredited nursing program Current Oklahoma LPN license (required) Current CPR certification (required) 2+ years of home health, palliative care, or related experience (preferred) Strong communication skills and a genuine passion for patient-centered care Valid driver's license and willingness to travel to patient homes in the area Company car provided for all work-related travel
