Job Description

The Manager Inpatient Case Management opportunity in northwestern New Mexico is an exciting chance for an experienced RN leader to step into a highly visible role with a respected healthcare organization known for its commitment to quality and patient-centered care. The Manager Inpatient Case Management reports directly to the Vice President and will oversee approximately 25 FTEs within the Case Management and Care Coordination department. The Manager Inpatient Case Management will work closely with interdisciplinary leadership, nursing teams, physicians, social services, and post-acute partners to ensure smooth patient progression and safe discharge planning. This role calls for a leadership style that balances transformational guidance with servant leadership principles, while remaining very hands-on in daily operations.

The Manager Inpatient Case Management will lead a dedicated Case Management team focused on patient flow, resource utilization, discharge planning, and care coordination throughout the acute care stay. The Manager Inpatient Case Management will oversee a fast-paced department that utilizes Cerner and WellSky systems while partnering with experienced clinicians and support teams who are passionate about delivering exceptional care. The culture within the department is collaborative, accountable, and highly team-oriented, with leaders who genuinely support professional growth and development.

The Manager Inpatient Case Management will provide leadership across both the clinical and operational aspects of the department, including staff mentoring, education, quality improvement initiatives, and competency development. The Manager Inpatient Case Management will also oversee budgeting, staffing, policy implementation, utilization review standards, and relationships with post-acute agencies to support appropriate length of stay and safe patient transitions. This role will play an important part in strengthening department operations while continuing to elevate patient care outcomes across the organization.
