Job Description
Job Summary We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to serve as a QA and Home Health Care Coordinator. In this vital role, you will ensure the delivery of high-quality patient care while coordinating home health services with precision and compassion. Your expertise in nursing, physiology, and medical documentation will help maintain compliance with healthcare standards and foster positive patient outcomes. Duties Conduct thorough quality assurance reviews of patient care plans, ensuring adherence to healthcare regulations and best practices. Coordinate home health visits by scheduling, confirming appointments, and communicating effectively with patients, families, and healthcare providers. Monitor patient progress through detailed medical documentation, updating records accurately while maintaining strict HIPAA confidentiality standards. Assess patient needs by applying comprehensive knowledge of anatomy, physiology, and medical terminology to develop personalized care strategies. Collaborate with interdisciplinary teams to optimize care delivery, troubleshoot issues, and improve overall service quality. Educate patients and their families on health management techniques, medication protocols, and safety procedures within the home environment. Ensure compliance with all relevant healthcare policies, including HIPAA regulations and medical recordkeeping standards. Qualifications Valid LVN license in good standing with relevant state licensing boards. 1 year + of proven experience in patient care, nursing, or home health services. Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and documentation. Familiarity with HIPAA regulations to protect patient privacy and confidentiality at all times. Excellent skills in medical documentation and record management to ensure precise recordkeeping. Ability to communicate effectively with diverse populations including patients, families, and healthcare teams. Detail-oriented mindset with strong organizational skills to manage multiple priorities efficiently.
