Clinical documentation Home health Care plans Full Job Description Job Overview We are seeking a dedicated and detail-oriented Home Health LVN QA Care Coordinator to join our dynamic healthcare team. In this vital role, you will oversee the quality assurance processes within our home health services, ensuring that patient care meets the highest standards of safety, compliance, and excellence. Your expertise in nursing, patient assessment, and post-acute care will drive continuous improvement in care delivery, fostering positive patient outcomes and supporting our mission to provide compassionate, comprehensive home health services. This position offers an exciting opportunity to influence care quality while working closely with interdisciplinary teams to uphold regulatory standards and enhance patient satisfaction. Duties Conduct thorough reviews of patient records, medical documentation, and care plans to ensure accuracy, completeness, and compliance with HIPAA regulations and healthcare standards. Perform detailed audits of home health visits, verifying that nursing assessments, vital signs, discharge planning, and diagnostic evaluations are properly documented and aligned with clinical protocols. Collaborate with nurses, case managers, physicians, and other healthcare professionals to address discrepancies or deficiencies in documentation or care processes. Monitor utilization management activities to optimize resource use while maintaining high-quality patient care. Assist in the development and implementation of quality improvement initiatives focused on infection control, patient safety, and clinical outcomes across diverse settings such as outpatient clinics or inpatient units. Provide education and support to nursing staff regarding medical terminology, physiology knowledge, and best practices in acute care, hospital medicine, or behavioral health. Ensure all documentation adheres to regulatory requirements and organizational policies; prepare reports for audits or compliance reviews. Support discharge planning efforts by reviewing post-acute care arrangements and ensuring seamless transitions for patients across settings including PICU or Geriatrics populations. Qualifications Valid LVN license with current state certification required; additional certifications in case management or utilization review are a plus. Proven experience in home health nursing or post-acute care environments; familiarity with outpatient clinics or inpatient hospital settings preferred. Critical care experience including ICU, emergency room experience, or trauma center background highly desirable. Strong knowledge of medical terminology, physiology, HIPAA regulations, and diagnostic evaluation procedures. Experience with medical records management and patient assessment techniques across diverse populations such as pediatrics (PICU), geriatrics, or behavioral health patients. Excellent attention to detail with the ability to analyze complex clinical data efficiently. Exceptional communication skills for collaborating with multidisciplinary teams and providing education on clinical standards. Ability to adapt quickly to changing priorities while maintaining a positive attitude focused on quality improvement.
