Job Description

Job Overview We are seeking a highly organized and proactive PCP Coordinator to join our team. In this vital role, you will serve as the primary point of contact for coordinating member support plans and goals with case managers, guardians, service providers, DHHS representative, and of course the members as well. Your role will include ensuring seamless communication and efficient management of member records and authorizations of services. The ideal candidate will possess strong knowledge of the Person Centered Planning process, HCBS, quality assurance measures, Atrezzo, Evergreen, and Developmental Services within OADS. Duties Manage the process of obtaining new authorizations for services, and managing renewals of services. Develop working with members in order to contribute , quarterly, and monthly written support summaries. Work directly with multiple agencies to ensure uninterrupted ongoing authorizations. Maintain HCBS quality assurance tracking programs. Maintain accurate and detailed member records in compliance with HIPAA regulations and HCBS standards. . Complete all necessary tasks for intake of new members. Support the main office by handling administrative tasks such as data entry, filing, record keeping, and documentation updates. Skills Strong understanding of MaineCare Sections 21 and 29 authorization processes. Proven experience working with Microsoft. Excellent organizational skills with the ability to prioritize tasks efficiently in a fast-paced setting. Familiarity with HCBS standards to ensure compliance with healthcare quality measures. Effective communication skills to liaise confidently with members, guardians , case managers, providers, and State representatives. Ability to navigate electronic records systems accurately and securely. Detail-oriented mindset with a commitment to maintaining confidentiality and accuracy in all documentation.
