06/23/2025 Job Summary The Family Specialist plays a vital role in supporting youth and families through direct in-home services, with a dual focus on implementing strengths-based interventions and expanding the agency's presence in a new service area. In addition to providing individualized support as part of the Child and Family Team (CFT), the Family Specialist will actively participate in community outreach and marketing efforts to raise awareness of the Wraparound program and help connect eligible families to services. This position is ideal for someone who is passionate about building trust with families, supporting high-fidelity Wraparound implementation, and fostering meaningful relationships in the community to help launch services in a new region. Key Functions, Accountabilities, and Related Duties Engage children/youth and their families with authenticity and respect to support them in achieving their vision of success. Deliver services in alignment with the Plan of Care, utilizing a strengths-based, individualized approach informed by the Fear to Love model. Act as a supportive, consistent presence in the home, modeling emotional regulation strategies and helping caregivers understand youth behaviors. Support children, youth, and parents in strengthening their connection to one another and their community. Reframe challenges into growth opportunities and recognize progress, no matter how small. Facilitate skill-building activities that promote family participation, healthy routines, and emotional well-being. Maintain accurate and timely documentation of services provided, including daily notes and reports to the CFT. Ensure the safety and supervision of children during visits, while helping them engage in structured, enriching activities. Adhere to all confidentiality, HIPAA, and risk management standards. Community Outreach Responsibilities Conduct targeted community outreach to raise awareness of Wraparound services among referral sources, families, and community partners. Building relationships with schools, community centers, nonprofits, and healthcare providers. Represent the agency at community events, meetings, and informational sessions to promote the availability and benefits of Wraparound support. Support the development and distribution of marketing materials and outreach communications tailored to local audiences. Work collaboratively with the Wraparound team to assess and respond to emerging community needs and adjust outreach strategies accordingly. Document outreach efforts and share updates during staff meetings to support program growth and family engagement strategies. Qualifications High school diploma or equivalent required; Bachelor's degree in social work or related field preferred. Experience working in home- and community-based services or Wraparound preferred. Knowledge of community-based resources and willingness to actively engage with local networks. Ability to connect with diverse populations and communicate effectively with children, families, professionals, and community members. Strong interpersonal, written, and verbal communication skills. Comfort with public speaking and community engagement. Must possess a valid driver's license, current auto insurance, and a reliable vehicle. Cleared criminal background check and completion of required trainings (e.g., Mandated Reporter, Sexual Harassment Prevention). Demonstrates cultural humility, equity-focused practice, and a commitment to inclusive service delivery. Physical Demands Regularly required to drive, sit for extended periods, and engage in in-home and community-based service activities.
