Community Engagement Case Manager Alternative Homes for Youth Denver, CO Job Details Full-time $50,000 - $60,000 a year 2 hours ago Benefits Health insurance Dental insurance 401(k) Paid time off 401(k) matching Life insurance Qualifications Youth engagement programs Bachelor's degree Case management Adolescents Working with youth Community resource coordination Behavioral health Full Job Description Job Overview Alternative Homes for Youth (AHFY), is a residential treatment center in Greeley, Colorado, serving adolescent males ages 15-20 involved in the juvenile justice system. We are seeking a passionate and proactive Community Engagement Case Manager to join our dynamic team. In this vital role, you will provide community case management services and work alongside Alternative Homes for Youth's clinical team to help ensure consistent youth access to the community to promote meaningful prosocial reintegration.
Duties :
  • Assist youth with identifying and completing community service and/or restorative justice projects
  • Assist youth with community shopping and budgeting
  • Assist with youth intakes and discharges as needed
  • Act as the facility's religious and spiritual liaison and assist youth in connecting with appropriate resources.
  • Coordinate donations with outside agencies
  • Oversee youth community engagement requests and document as needed
  • Plan and organize regular community group activities
  • Ensure consistent access to 12-step meetings
  • Assist youth in identifying prosocial interests and activities in the community
  • Assist youth in identifying and registering for skill-building activities
  • Attend facility staff meetings.
  • Organize and run facility's unit council at a minimum of two times per month and report to the Performance Quality Improvement (PQI) team youth requests, comments, and concerns.
Pay:
$50,000.00 - $60,000.00 per year
