The Opportunity This is not a typical sales position. We are looking for a motivated, compassionate, and entrepreneurial professional who enjoys building relationships, creating opportunities, and making a meaningful difference in the lives of others. You will serve as an ambassador for Life Foundation Home Care, developing partnerships throughout the healthcare and senior care community while helping families access trusted care solutions. Whether your background is in business development, social work, case management, healthcare marketing, community outreach, or patient advocacy, this role offers the opportunity to leverage your skills while helping grow an organization that truly makes an impact. What You'll Do Build and maintain relationships with referral partners, including: Hospitals Skilled nursing facilities Rehabilitation centers Assisted living and independent living communities Physicians and physician groups Social workers and case managers Hospice agencies Elder law attorneys Financial planners Community organizations and faith-based groups Generate qualified client referrals and new business opportunities Conduct in-person visits, presentations, and educational meetings Attend networking events, healthcare conferences, and community programs Identify and develop new partnership opportunities Maintain accurate referral and outreach records Collaborate with clinical and operations teams to ensure an exceptional client experience Serve as a trusted resource for families seeking care solutions Who We're Looking For The ideal candidate is: Self-motivated and results-oriented Comfortable working independently and managing their own schedule Passionate about helping seniors and families An excellent communicator and relationship-builder Professional, polished, and dependable Organized and accountable Comfortable networking and meeting new people Driven to achieve goals while maintaining a service-first mindset Preferred Backgrounds We welcome candidates from a variety of professional backgrounds, including: Social Work Hospital Case Management Care Coordination Discharge Planning Home Care, Home Health, or Hospice Marketing Senior Living Admissions Healthcare Business Development Community Outreach Patient Advocacy Relationship Management Candidates with established healthcare referral relationships are strongly encouraged to apply.
