Job Description

Job Title:
Provider Relations Manager (Community Outreach, Medicaid)
Locations:
Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster, Westchester •you must be open to traveling to anyone of these locations as needed for community events Hybrid work - remote with local travel up to 50% Ideal is 3 days onsite with provider and 2 days remote •Must have a reliable car to get around, cannot take public transportation•

Overview:
You'll lead strategic outreach and referral development across New York, focusing on Community Supports. You'll engage healthcare providers, case managers, and community organizations to drive awareness and referrals for programs. You'll also collaborate with internal teams to support member enrollment, patient navigation, and data-informed quality improvements. This role blends relationship building, sales strategy, and community health impact. You'll be measured by your ability to drive qualified referrals, increase program adoption, and support access to care for vulnerable populations.
Key Responsibilities:
Build and maintain relationships with healthcare providers, health plans, and community-based organizations, serving as the primary point of contact Drive engagement and referral growth through consistent outreach, follow-up, and partner support Conduct in-person and virtual meetings, presentations, and educational events to communicate program value and increase adoption Support provider onboarding and assist patients and partners through referral and enrollment workflows Connect with members to educate on the program & resources available to them Collaborate with clinical, operational, and care teams to ensure coordinated care and a seamless partner experience Plan and execute community partner events Track outreach activity, referrals, and performance metrics in CRM/EHR systems Gather provider feedback and market insights to inform strategy and strengthen competitive positioning
Required Skills:
Bachelor's degree required (Public Health, Nutrition, or related field preferred) 3+ years of experience in sales, outreach, or healthcare engagement Knowledge of NY State Medicaid & programs Ideally experience at commercial plan in provider relations/network Strong communication, CRM/EHR proficiency, and cultural humility Ability to work independently • We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state, and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO), and the California Fair Chance Act (CFCA). • As a job position within our Care Management division, a successful completion of a background check may be required as a condition of employment.
