About the Role We're looking for a people-first, relationship-driven professional to join our team as a Community Outreach & Partner Relations Manager . This role is ideal for someone who loves connecting with others, building trust in the community, and making a meaningful impact—while also helping grow a mission-driven healthcare business. You'll serve as the face of GentlePath in the community, developing partnerships and helping families access the care they need. What You'll Do Build Meaningful Community Relationships Develop and nurture referral partnerships with hospitals, rehab centers, assisted living communities, and local organizations Connect with social workers, case managers, and healthcare professionals as a trusted resource Attend community events, networking groups, and healthcare meetings Support Families & Referral Partners Act as a friendly, reliable point of contact for partners and prospective clients Help guide referrals through a smooth and supportive onboarding process Follow up consistently to ensure a positive experience Drive Growth (in a Relationship-First Way) Identify new opportunities to expand GentlePath's presence in the Houston area Share information about our services through conversations, presentations, and outreach Track partnerships and referral activity to help meet growth goals What We're Looking For Experience in healthcare, home care, senior living, or community outreach Strong relationship-building and communication skills A natural connector who enjoys meeting people and building trust Self-motivated and comfortable working independently in the field Familiarity with the Houston healthcare/community landscape is a plus Reliable transportation and willingness to travel locally Why Join GentlePath?
