Job Description

Overview We are seeking a dynamic and passionate Community Liaison Director PHP to lead our community engagement initiatives and foster meaningful relationships within diverse communities. This role offers an exciting opportunity to drive strategic outreach, enhance public relations, and promote our organization's mission through innovative communication and partnership development. As a key leader, you will coordinate with various stakeholders, manage social media platforms, and oversee volunteer programs to strengthen community ties and support organizational growth. Role Description The Community Liaison Director - PHP is a full-time, on-site position based in Phoenix, AZ. In this highly visible role, you'll build and strengthen relationships throughout the community, lead strategic outreach efforts, and support home care initiatives that advance the organization's mission. You will actively engage local stakeholders, deliver consistently exceptional customer service, and champion programs and services that expand access, awareness, and impact. Working closely with internal teams, you'll help achieve strategic objectives while deepening trust and collaboration across the community. Acts as the key connection between hospitals and community-based mental health professionals and agencies across an evolving geographic region. Qualifications Minimum one-year experience working in-patient or outpatient psychiatric facilities. Strong Community Engagement and Community Outreach skills, with experience fostering partnerships and building relationships. Effective Communication skills, including written, verbal, and interpersonal communication for diverse audiences. Proven expertise in Customer Service and working with various stakeholders to provide exceptional service and support. Knowledge and experience in Home Care practices and services, with the ability to advocate for community needs. Leadership abilities to oversee outreach initiatives and direct programs successfully. Knowledge of Phoenix, AZ community dynamics is a plus. A Bachelor's degree in Social Work, Healthcare Administration, Communications, or a related field is preferred. Ability to work effectively within a dynamic team and adapt to the needs of a diverse community. Job-Specific Duties Build and grow referral relationships (physicians, allied health, community partners); identify new sources. Provide community education, referral support, and coordinate tours. Maintain strong communication with physicians, community providers, nursing, and program staff; partner with the Program Director. Support patient interventions as needed and follow up with admitted clients to sustain engagement. Complete at least 20 in-person referral contacts/week and stay current on community issues/trends; report updates daily to the Program Director. Understand and support the outreach workflow (intake, screening, insurance verification, admission, follow-up). Attend weekly CHC unit visits and weekly staff/treatment team meetings. Manage priorities, return calls within 1 hour, and escalate significant issues promptly to the SVP. Coordinate transportation, complete documentation/insurance verification, and provide pre-admission patient orientation (including copays/deductibles). Perform other duties as assigned.

$68,640.00 - $102,960.00 per year
