Program Overview:
Connecting D.O.T.S (Developing and Orchestrating Training Systems) is an innovative program that uses a community-driven access model to empower, educate, and train families of Early Start, school-aged, and adult Black and Hispanic individuals with IDD from disenfranchised communities, and to build a collaborative ecosystem of responsive service providers and referral sources. This program utilizes a culturally and linguistically relevant approach to provide advocacy tools, bridge service gaps, and improve access to care. Key components include community workshops, resource fairs, professional development, and a digital platform called "UNI Train", structured around a 4-phase D.O.T.S. approach.
Position Overview:
The Community Liaison serves as a trusted connector between families, community organizations, service providers, and the Connecting D.O.T.S. (Developing and Orchestrating Training Systems) program. This position is responsible for building relationships, conducting outreach, facilitating community engagement activities, and increasing awareness of resources and services available to Black and Hispanic individuals with intellectual and developmental disabilities (IDD) and their families. Using a culturally and linguistically responsive approach, the Community Liaison helps families navigate systems of care, access educational opportunities, and connect with community resources. The position plays a key role in supporting the program's mission to reduce service disparities, strengthen community partnerships, and empower families through education, advocacy, and training.
Essential Duties & Responsibilities:
Community Outreach & Engagement Conduct outreach to families, schools, community organizations, faith-based groups, and service providers to increase awareness of the CDOTS program. Build and maintain strong relationships with community stakeholders and referral partners. Represent the organization at community meetings, resource fairs, workshops, and networking events. Promote program services and recruit participants for training opportunities and events. Assist with the planning, coordination, and implementation of community workshops and resource fairs. Family Support & Resource Navigation Provide information and referrals to families seeking services and supports for individuals with IDD. Assist families in accessing community resources, educational opportunities, and advocacy tools. Support participant engagement throughout the CDOTS 4-phase D.O.T.S. model. Maintain regular communication with families to encourage participation and successful program outcomes. Community Partnership Development Collaborate with schools, regional centers, service providers, community-based organizations, and other stakeholders. Identify opportunities to expand partnerships that support program goals and increase community access to services. Participate in collaborative meetings and community coalitions as assigned. Data Tracking & Grant Compliance • Responsible for maintaining meticulous, accurate records of time, effort, and programmatic metrics to support quarterly and annual grant reporting requirements as mandated by the funding administrator • Research available grant opportunities that would generate revenue for UNI services Program Support & Documentation Maintain accurate records of outreach activities, participant engagement, referrals, and program outcomes. Assist with data collection and reporting requirements. Support the promotion and utilization of the UNI Train digital learning platform. Contribute to the development of outreach materials and community education resources. Participate in team meetings, trainings, and professional development activities. Perform other duties as assigned.
Requirements & Qualifications:
Bachelor's degree in Social Work, Human Services, Public Health, Education, or a related field. Experience in community outreach, family support, social services, education, healthcare, or a related field. Valid driver's license and reliable transportation § CPR / First-Aid Certification (or willingness to obtain) Ability to travel throughout the service area and attend community events. Successful completion of background check and criminal record clearance.
Preferred Qualifications:
Experience working with individuals with intellectual and developmental disabilities and their families. Bilingual proficiency in English and Spanish strongly preferred. Experience facilitating workshops, presentations, or community events.

Working Conditions:

$24.50 - $30.00 per hour
