We are seeking a motivated and relationship-driven Home Health Liaison to join our growing team and help connect patients, families, physicians, hospitals, and community partners with the care they need. Position Summary The Home Health Liaison serves as the primary representative of Kamelya Home Health within the community. This role is responsible for developing and maintaining referral relationships, educating healthcare professionals about home health services, increasing patient referrals, and supporting the admission process. The ideal candidate is an energetic professional with strong communication skills, a passion for patient advocacy, and a proven ability to build lasting partnerships. Essential Duties and Responsibilities Develop and maintain referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, and other healthcare professionals. Promote Kamelya Home Health services through regular in-person visits, presentations, and community outreach activities. Identify and pursue new business opportunities to increase referrals and market share. Collaborate with leadership to develop and implement strategic marketing plans and growth initiatives. Prepare and present monthly activity reports, sales goals, and action plans. Represent the company at networking events, healthcare conferences, community programs, and professional organizations. Educate referral sources and community partners on home health eligibility, services, and regulatory requirements. Assist patients and families in navigating the referral and admission process. Address referral source concerns promptly and professionally while maintaining positive relationships. Maintain accurate documentation of marketing activities, referral activity, and business development efforts. Stay informed on healthcare industry trends, Medicare guidelines, and home health regulations. Qualifications Bachelor's degree in Marketing, Business Administration, Healthcare Administration, or a related field preferred. Minimum of two (2) years of experience in home health, healthcare sales, patient navigation, case management, or a related healthcare setting. Strong relationship-building, communication, and presentation skills. Ability to work independently and manage multiple priorities. Knowledge of Medicare, home health regulations, and referral processes preferred. Valid driver's license with a clean driving record. Reliable transportation with current automobile insurance and registration. Ability to attend a two-day orientation program.
