Job Description
Mindful Care helps people access same and next-day mental health care, eliminating the long wait times that keep so many people from getting the support they need. We operate across multiple states and are growing our presence in Illinois and Michigan. We are looking for a community liaison to build and deepen referral relationships across both states. This is a relationship-first role. The right person knows the behavioral health and healthcare landscape in this region, has built trust with the providers and organizations that move people toward care, and is ready to put that network to work for a mission worth believing in. You might come to this role from a social work background. You might come from provider relations or healthcare business development. What matters most is that you have real relationships in this market and the skill to build more. Who we are looking for The ideal candidate brings one or both of the following: LCSW or LMSW, who has built meaningful relationships and is ready to step into a community-based partnerships role An experienced community liaison or provider relations professional with 2+ years of behavioral health outreach or business development experience in Illinois and/or Michigan What you will do Build and grow referral relationships with hospital discharge planners and care coordinators, primary care providers, therapists, healthcare organizations, and community partners across Illinois and Michigan Conduct outreach through meetings, presentations, conferences, and networking events Educate partners on Mindful Care's services, access model, and referral pathways Identify new partnership opportunities and develop a regional strategy over time Collaborate with the marketing and operations teams to support growth Track outreach activity and partnership engagement in our CRM What you will bring LCSW or LMSW strongly preferred, or equivalent experience in behavioral health business development An existing network of referral relationships in Illinois and/or Michigan is preferred 2+ years of experience as social worker, community liaison, provider relations, or business development in a healthcare or behavioral health setting Comfort managing a regional territory independently, with willingness to travel across both states as needed Strong relationship-building and communication skills Why Mindful Care A real opportunity to shape regional strategy in a territory with genuine room to build Mileage reimbursement at the IRS rate Medical, dental, and vision coverage 401(k) with employer contribution PTO, paid holidays, and parental leave Mission-driven work directly connected to getting people into mental health care faster than they could access it anywhere else All offers to candidates will ultimately be based on that candidate's individual experience and skillset.
