Job Description

Eden Senior Care, founded in 2016, is a growing Healthcare Management company, focused on managing and operating Skilled Nursing, Rehabilitation and Assisted Living communities in Minnesota, Wisconsin, and Ohio. Our mission with each community is to support their success by providing strong leadership, corporate support, and the resources and tools to realize their goals. We are looking for a Regional Hospital and Community Liaison who can support Marketing and Admissions Coordinators with screening referrals for our skilled nursing facilities in Minnesota. The candidate must be a self-starter, self-driven and ambitious.
Position:
Regional Hospital and Community Liaison Qualifications:
With more than 2 years in admissions and marketing, business development, or relevant experience in Skilled Nursing and/or Assisted Living setting. With proven success in census development. Prior Regional experience is preferred (multi-site) Willing to travel 80% of the time with 1 day per week in the office.
Responsibilities:
Our Admissions Team Staff establish a presence by representing our facilities to all area hospital discharge planning teams for screening, evaluating, and recommending patients for Short Term Rehabilitation or Long-Term Care at one of our premier healthcare facilities. Establishes and maintains positive working relationships with current and potential referral and payer sources. Provide insights to Executive Management about appropriate strategies to grow the Census, which may include the introduction of Care Specialties, Operational Changes, etc. Directs all daily operations including providing direct oversight of the establishment and implementation of referral initiatives. Overall admission process oversight Admissions team management Census calls and conversion management. New hire interviewing and training Admission software, systems know-how, management and training Back-up coverage models Financial verification management Insurance information, process, authorization, and portal management and training Transition of care communication CRM oversight, management, and training. #INDSCLEAD
