This position focuses on increasing awareness of HOPES services throughout the community, developing referral partnerships, engaging eligible families, and increasing program enrollments. The specialist serves as a key connector between the HOPES program and community organizations such as schools, hospitals, clinics, childcare centers, churches, shelters, and other agencies serving children and families. The position is heavily outreach and engagement focused and does not maintain an ongoing caseload. However, the specialist will assist families through the enrollment process, complete assessments and documentation as needed and may occasionally assist with direct service support when operationally necessary. The Community Liaison & Family Engagement Specialist will obtain Community Health Worker (CHW) certification within one year of hire and maintain a strong understanding of HOPES services, eligibility requirements, and community resources. Key Responsibilities- Outreach & Referral Development Develop and maintain strong referral relationships with schools, hospitals, clinics, physician offices, childcare providers, churches, shelters, and other community organizations serving families. Build and strengthen a consistent referral pipeline for the HOPES program. Conduct outreach activities to increase awareness of HOPES services throughout the community. Represent the HOPES program at community events, health fairs, coalition meetings, outreach events, and networking opportunities. Educate community partners and families about HOPES services, eligibility requirements, and enrollment processes. Conduct follow-up communication with referral partners to maintain engagement and collaboration. Identify underserved areas and opportunities for increased family engagement and enrollment. Key Responsibilities- Family Engagement & Enrollment Assist eligible families through the intake and enrollment process for the HOPES program. Conduct Intake1 assessments and collect all required eligibility documentation. Ensure enrollment documentation is accurate, complete, and entered into Intake1 within required timelines. Maintain communication with referred families to encourage participation and reduce barriers to enrollment. Connect families with appropriate HOPES services and community resources. Support HOPES events, parent engagement activities, and Family Resource Center events as needed. Occasionally assist with direct service activities or participant support when operational needs arise. Key Responsibilities- Program Support & Documentation Maintain accurate records of outreach activities, referrals, enrollments, and community contacts. Track outreach effectiveness and referral trends. Participate in staff meetings, trainings, and HOPES program activities. Obtain Community Health Worker (CHW) certification within one year of hire. Perform other duties as assigned. Performance Expectations & Metrics The Community Liaison & Family Engagement Specialist is expected to meet the following minimum performance standards:
Enrollment Goals:
Build up to and maintain a minimum of 18 HOPES enrollments per month.
Outreach Activity:
Participate in a minimum of 6 outreach or community engagement activities per month.
Referral Development:
Develop and maintain active referral relationships with community organizations and partners serving families with young children.
Documentation:
Ensure all enrollment documentation and outreach records are accurate, complete, and entered into Intake1 within established timelines.
Community Engagement:
Maintain a visible and professional presence in the community while promoting the HOPES program and Project Unity mission. Required Qualifications Associate degree or higher in a health or human services field OR Two (2) years of direct service, outreach, enrollment, or community engagement experience in a health or human services setting Strong interpersonal and relationship-building skills Strong verbal and written communication skills Ability to work independently and manage multiple priorities Strong organizational and documentation skills General knowledge of community resources and family support systems Valid driver's license, car insurance, and reliable transportation
Pay:
$44,000.00 - $47,000.00 per year
