We are seeking a motivated, relationship-driven Community Liaison to expand our referral network and strengthen partnerships throughout the community. Job Responsibilities Build, develop, and maintain strong relationships with referral sources. Meet and exceed monthly patient admission goals. Coordinate with hospitals, inpatient rehabilitation facilities, skilled nursing facilities, physician offices, and other healthcare providers to facilitate patient care and secure referrals. Conduct regular in-person visits to referral partners within the assigned territory. Represent Boundless Home Health professionally at networking events and community functions. Be available for phone calls and attend in-office meetings as required. Maintain accurate records of referral activities and provide regular updates to leadership. Qualifications Minimum of 2 years of Home Health marketing experience required. Established book of business and existing referral relationships within the Home Health industry is required. Excellent communication, networking, and relationship-building skills. Self-motivated with the ability to work independently. Valid driver's license and reliable transportation.
