Job Description

Job description: Med Team, Inc. is seeking a dedicated PAS Supervisor/Liaison to ensure the quality and safe delivery of Primary Home Care services. In this essential role, you will assist in coordinating and providing personal attendant services that align with our agency's philosophy and established standards.
Key Responsibilities:
Supervisory Oversight :
Provide guidance and support to Personal Care Attendants within the Primary Home Care department.
Service Monitoring :
Ensure that client services are executed according to task assignments and adhere to agency policies.
Onsite Assessments :
Periodically assess the quality of services provided at clients' homes.
Client Communication :
Engage with clients and their families to ensure satisfaction with services.
Issue Reporting :
Report any concerns or issues to the appropriate agency staff for resolution.
Training and Orientation :
Orient Personal Care Attendants to new client assignments, explaining tasks and schedules while assessing their competency.
Performance Evaluation :
Evaluate the job performance of attendants and provide constructive feedback.
Documentation and Compliance :
Complete necessary documentation and supervisory visit reports, including all relevant information regarding client services.
Client Coordination :
Notify family members or physicians of significant changes in a client's health or safety and coordinate care with agency programs.
Qualifications/Experience:
Education :
Minimum of a high school diploma or G.E.D.; completion of two years at an accredited college or university preferred (substitutable with relevant supervisory experience).
Experience :
2 years of supervisory experience in a healthcare facility, home care agency, or community agency is required; 2 years of experience in home and community care is preferred.
Language Skills :
Bilingual proficiency is required.
Licenses :
Valid State Driver's License with a good driving record.
Physical Requirements:
Sufficient visual and hearing ability to comprehend written and oral communication. Capacity to deal effectively with stress. Ability to use hands for gripping and feeling, and to reach with hands and arms. Capability to bend, stoop, kneel, and stand for extended periods.

Full-time Projected Total Compensation:
$42,000.00 - $50,000.00 per year
