The primary purpose of this position is to coordinate the admissions of new and returning residents from home, hospital, or other health care settings and to maintain the facility at its maximum capacity. The position also entails a marketing component to hospitals and other potential clientele for the purpose of promoting long term, transitional, and exceptional care offered by the facility.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. 1. A high school diploma or a degree in Social Work is preferred. 2. Some business training is also preferred 3. Equivalent related work experience in LTC setting; office management experience preferred. 4. Caring person with a desire to work with the frail and elderly. 5. Practical knowledge of medical terminology and record keeping preferred. 6. Must be physically and mentally capable of performing routine job duties. 7. Must have compassion, tolerance, and understanding for the elderly. 8. Good organizational, planning, and marketing skills. 9. Knowledge of hospital discharge planning and policies as it relates to the long-term care population. 10. Knowledge of JCAHO, OBRA, MDPH, and MDPA regulations relevant to admissions, discharges, and resident rights
ESSENTIAL FUNCTIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Support the facility's philosophy of care and strive to achieve its goals and objectives. B. Receive all inquiries regarding prospective residents from health care facilities and private individuals to determine an admission date or to schedule an appointment. C. Conduct tours of the facility to prospective residents and/or family members. D. Complete the pre-admission assessment (inquiry) form and present information regarding a prospective resident to the Executive Director and Director of Nursing for evaluation to determine facility and medical appropriateness. E. Inform necessary departments of the arrival of new residents and coordinate the admission process as outlined in the facility policy. F. Complete the admissions packet and necessary documentation for every admission (regardless of source or payor status): Obtain a "DON", MDPH Form #2536, and OBRA Form for appropriate residents. Secure deposit from resident or the responsible party. Obtain signatures of resident and/or the responsible party on the contract and all other admission documents. Complete Social Security representative payee applications for private, Public Aid and Public Aid Pending residents. Determine Medicare eligibility, days remaining, and communicate pertinent information to the business office staff. Complete parts of the MDS form relevant to admissions. Inform resident and family of Advanced Directives and "Do Not Resuscitate" orders. Inform resident and family of rules of spousal impoverishment and prepaid funeral arrangements. Obtain copy of Medicare, Social Security, insurance, and other cards Complete the Resident Admission Assessment Section of the MDS form upon admission, quarterly, annually and/or in cases of significant change. Submit statistical information regarding long term care inquiries and admissions to the Executive Director on a monthly basis G. Coordinate long term admissions with area hospital case managers and social workers and submit to the Executive Director for final approval. H. Complete daily census sheets for presentation at the admission meeting and monthly reports on census and resident sourcing for the Executive Director. I. Facilitate daily admissions meetings and provide data regarding resident/family background, family involvement, resident behavior, payer status, etc. to staff in attendance and coordinate room changes as needed. J. Maintain communication with hospital social workers/discharge planners, families, and nursing staff to coordinate resident re-admissions. K. Greet new resident and family upon admission. L. Perform the following Public Relations duties to the Referral Sources Periodically send thank you letters to the social workers and discharge planners at the hospital. Periodically send thank you letters to the physicians. After fourth day, send letter to social worker, discharge planner, and physician as to the status of the patient M. Send sympathy cards to bereaved families. N. Maintain current data base regarding hospital social workers, discharge planners, case managers, and discharge formats. O. Maintain information of current community resources and assist families in securing needed services within the community. P. Attend Continuity of Care meetings, public speaking events, and health fairs as needed with approval of the Executive Director. Q. Develop and maintain working relationships with hospital and senior center social workers, discharge planners, and case managers to stimulate referrals to general admission as well as monitor satisfaction of referring parties to the facility. R. Visit one marketing group per week with the Marketing Director to area hospitals, complete quarterly marketing visits, and deliver monthly report to the Executive Director. S. Work with Director of Resident and Family Services and Patient Accounts Auditor in securing Public Aid approval for appropriate residents. T. Attend Medicare, Department Head, Marketing, Resident Council and Performance Improvement meetings and others as deemed appropriate U. Act as a backup in any aspect of TCU admission, i.e. inquiry, assessment, admission packet, and family support. V. Work as the weekend supervisor as part of the department head monthly rotation. W.
