Job Description
We are seeking a passionate and driven In-Home Marketing Specialist to represent our in-home support services within the Intellectual and Developmental Disabilities (I/DD) community. In this field-based role, you will serve as the face of our organization — building relationships with referral sources, educating stakeholders about our services, and driving census growth for individuals who require community-based support. This is a mission-driven position for someone who understands the I/DD landscape and is motivated by meaningful work. Required qualifications Minimum 2 years of experience in healthcare, human services, or I/DD marketing/outreach Working knowledge of Medicaid waiver programs, home and community-based services (HCBS), and I/DD systems Proven ability to build trust-based relationships with clinical and administrative referral partners Valid driver's license and reliable transportation for daily field travel Strong written and verbal communication skills; comfortable presenting to small and large groups Bachelor's degree in social work, health administration, marketing, or a related field (or equivalent experience) Preferred qualifications Experience working with support coordination organizations or within a Medicaid waiver provider agency Existing professional network in the I/DD or behavioral health provider community Familiarity with person-centered planning, ISPs, and state developmental disability systems Bilingual (English/Spanish or other language) a strong plus CRM experience (Salesforce, HubSpot, or similar) Key responsibilities Conduct regular in-person outreach visits to physicians, neurologists, pediatricians, and hospital discharge planners to introduce and promote in-home support services for individuals with I/DD. Build and maintain partnerships with support coordination agencies, case managers, and Medicaid waiver coordinators to generate consistent referral pipelines. Develop relationships with rehabilitation centers, behavioral health providers, and skilled nursing facilities to identify individuals transitioning to community-based living. Represent the organization at community events, health fairs, and I/DD advocacy forums; serve as a brand ambassador in the broader disability services community. Track all outreach activity, referral sources, and conversion metrics using a CRM system; provide regular reporting to leadership on pipeline status and growth opportunities. Educate prospective referral sources on eligibility criteria, the intake process, waiver programs, and the scope of services offered, ensuring clarity and confidence in the referral relationship. Collaborate with internal intake, clinical, and operations teams to ensure smooth transitions for newly enrolled individuals and families.
