FT Days This employee is responsible for the discharge planning for patients receiving care on the acute mental health units. The Discharge Planner provides patients with a review of treatment options beyond V alle V ista H ealth S yst em . Education:
Bachelor's degree in social work or other social science is required Experience :
Six months to one year of experience working with a psychiatric in-patient population preferred. Additional Requirements :
Must be at least 21 years of age. Work Hours:
8:30 am - 5:00 pm Monday - Friday.
