MDS Assessment Nurse

Pontchartrain Health Care Centre, a long-term care, skilled nursing facility is seeking an LPN as MDS nurse to perform MDS assessments. Pay range is commensurate to licensure and MDS experience . Previous MDS experience is preferred but will train the ideal candidate; knowledge of the Point Click Care (PCC) system is preferred; RAC Certification is ideal. Responsibilities include but may not be limited to: Completion of the MDS/RAI process Coordination of all aspects of the RAI process while collaborating with the interdisciplinary team Participating in drafting, revision and review of resident care plans Participating in facility Quality Assurance/Performance Improvement (QAPI) initiatives Qualifications and skills are current; License is active with the Louisiana State Board Interested applicants please submit a resume online through this site.
