MDS Coordinator

Alliance Health at Coleman is recruiting for an RN Case Manager 32-40 hours per week. Assess and evaluate the quality of care being given to nursing facility residents per CMS guidelines. Ensure completion of MDS interviews and assessments following the RAI process. Develop a plan of care that has measurable goals collaborating with residents'/caregivers. Ensuring regulatory compliance of Medicare, Medicaid, and Managed Care by following federal and state guidelines. Coordinate with multidisciplinary team to ensure implementation of the residents' plan of care, assessments, and education. Conduct and attend meetings with the multidisciplinary team, caretakers, and families. Current/active Massachusetts Registered Nurse license RAC-CT certification preferred.
