Job Description

Job description: Job Overview We are seeking a dedicated and detail-oriented Licensed Vocational Nurse or Registered Nurse (LVN ) to serve in a MDS Nurse. This unique position plays a critical role in supporting both the clinical and regulatory functions of our skilled nursing facility. The ideal candidate will have a strong clinical background combined with a working knowledge of MDS documentation practices in accordance with federal and state guidelines. In this role, you will assist with the accurate and timely completion of MDS assessments and interdisciplinary care plans that help shape the quality of care we deliver. This is a vital position that contributes directly to our residents' health outcomes and our facility's compliance standards. Duties Conduct thorough assessments of patients' wounds and overall health status. Develop individualized care plans based on clinical assessments and patient needs. Collaborate with interdisciplinary teams to coordinate comprehensive patient care. Maintain accurate medical records, documenting assessments, treatment plans, and patient progress. Stay updated on best practices in wound management and participate in continuing education opportunities. Utilize knowledge of medical terminology, HEDIS standards, MDS systems, and physiology to enhance patient care.
MDS Nurse Responsibilities:
Assist with MDS assessments, including ARD scheduling, data collection, and documentation. Complete assigned sections of MDS, CAAs, and care plans. Support care plan meetings and ensure interdisciplinary coordination. Monitor medical records for documentation accuracy related to MDS coding. Participate in survey readiness and QAPI activities. Provide staff education on documentation and ADL scoring, in collaboration with the DON and MDS Nurse Requirements Valid nursing license with experience in mds preferred Strong background in patient care with a focus on pediatrics or hospice care. Familiarity with medical records management and documentation practices. Knowledge of medical terminology Excellent communication skills to effectively educate patients and collaborate with healthcare teams. Ability to work independently while managing multiple cases effectively. MDS certification a plus. MDS experience preferred.
