CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. A typical day includes:

Completing, auditing, and submitting Medicare, Medicaid, and PPS assessments (MDS 3.0) according to regulatory timelines. Collaborating with the Interdisciplinary Team (IDT) to develop and update individualized, comprehensive care plans. Reviewing clinical records and nursing documentation

Job Requirements:

Education :

Associate's degree

Experience:

LTC and MDS experience preferred

Skills:

Strong attention to detail

License:

LPN, LVN, RN

Physical:

Ability to sit or stand for extended periods, and move throughout the facility to monitor resident care.
