MDS Nurse

The Gardens & Guardians is seeking a detail-oriented and compassionate MDS Nurse to join our skilled nursing team. This position plays a vital role in ensuring accurate resident assessments, individualized care planning, regulatory compliance, and quality outcomes for our residents. Responsibilities:

Complete and coordinate MDS assessments accurately and timely Develop and update individualized resident care plans Ensure compliance with state and federal regulations Collaborate with interdisciplinary team members including nursing, therapy, dietary, and social services Monitor documentation to support quality care and reimbursement accuracy Participate in care plan meetings and quality improvement initiatives Maintain knowledge of Medicare, Medicaid, PDPM, and RAI guidelines

Qualifications:

Current Louisiana LPN license in good standing Previous MDS experience preferred Skilled nursing or long-term care experience preferred Knowledge of PDPM and Medicare/Medicaid regulations Strong organizational and communication skills Ability to work independently and as part of a team Experience with PointClickCare or other EMR systems is a plus
