Job Description
Medical Appointment Coordinator - Arc GLOW Responsible for the coordination of all medical appointments and all necessary communication and documentation as needed to link individuals who live in residential sites to proper medical care. Responsible for providing direct support as needed.
Essential Functions:
Responsible for assisting people supported in receiving needed/ desired medical services and communicating and documenting pertinent information as per program policy (i.e. medication administration, transportation to appointments). Provides medical supervision of people living in the residence, as needed. Reviews the MAR for refusals daily and sends information to the medical providers. Track injury/falls for the people of the residence. Sends monthly MAR renewals to the pharmacy with the appropriate changes. Ensures an adequate amount of medication is on site for each individual at all times. Provides communication to residential site staff to ensure physician and clinical orders are understood and followed. Makes recommendations to the CSRN and/or Health Care Provider to maintain and/or improve the overall health and wellness of individuals supported. Coordinates with the CSRN to assure follow up of Pharmacy Reviews. Reviews the medical charts to ensure labs, test results, etc. are completed. Assures that follow up is done within appropriate time frames. Attends weekly meetings with the CSRN as requested. Implements Life Plan and Staff Action Plan as developed by each person supported, family / advocate and Care Coordinator. Assists and supports each person in using community cultural and education resources, such as libraries, restaurants, stores, medical facilities, banks, churches, theater and social services as needed. Responsible for completing household tasks, including meal preparation, following a nutritional plan, grocery shopping, laundry, etc. as needed. Assists and supports people with interpersonal relationships in their home and community, and with medications if necessary. Complete required paperwork on daily / as needed basis. Ensures residential habilitation services are documented in accordance with agency and OPWDD requirements. Provide direct support as needed. Drives agency vehicles, when needed, and complete required transportation paperwork. Promotes cooperation and successful interaction by maintaining open and positive communication with people receiving services, their advocates, family members and other Agency staff members. Adheres to all program policies and procedures. Work holiday and weekend hours, as needed Become certified in medication and administration and maintain certification on an annual basis. Participates in corporate compliance, quality assurance and improvement activities as required. Demonstrates adherence to the DSP Code of Ethics. Other related duties as assigned.Nonessential Functions:

None exercisedKnowledge, Skills, and Abilities:
Requires good listening skills, excellent oral and written communication skills and good math and reading abilities. Basic computer skills required. Strong organizational skills and problem-solving skills, ability to evaluate and network. Experience working with individuals with developmental disabilities and/or elderly helpful. Must be a positive role model and possess both excellent interpersonal skills and leadership abilities. Must be able to work independently, as well as part of a team.Physical & Mental Requirements:
Be physically able to assist individuals in transferring from wheelchair to other seat when needed. May be required to prevent an adult from falling, assisting an adult to his/her feet, etc. (adults weighing up to 200 pounds, plus). Walk up and down stairs. Shovel and remove snow and ice from walkways and driveways when necessaryMinimum Qualifications:
High School Diploma or equivalent At least one year experience with individuals with developmental disabilities preferred. Certified Nursing Assistant or Certified Home Health Aide preferred Valid NYS driver's license with a safe driving record per agency policyWorking Conditions:
Will be required to use office equipment, i.e., phone, copier, fax in addition to household appliances, i.e., washer/dryer, stove/oven, refrigerator, etc. Work may require offsite travel to appointment / events / outings.Job Type:
