Job Description

Responsibilities:
The Medical Records Clerk manages, organizes, and maintains patient health records, ensuring accuracy, confidentiality, and accessibility for healthcare providers; Retrieve medical records for Providers, Patients, External sources and Staff; Ensure all records comply with HIPAA and other federal regulations, maintaining confidentiality and security of patient information. Closure of the medical records to include all required forms, signatures, etc. Clarion Psychiatric Center offers inpatient treatment for adults with moderate to severe psychiatric problems, inpatient crisis stabilization for children and adolescents, and a Partial Hospitalization Program for school-aged children. Our recently expanded 112-bed inpatient program has 4 units. Qualifications:
Job Description:
Responsibilities include but are not limited to chart assembly and analysis, coding using ICD-10-CM and DSM V, abstracting. Ensuring Medical Records Department is organized, assisting with performance improvement studies, filing, record review, responding to internal and external customer inquiries.
Job Specifications:

Education:
High School Graduate. Medical Records/HIM experience required. Completion of a Medical Coding course is required; certification as a Professional Coder (CPC) is required upon completion of Medical Coding course. •
To Scribe:
Valid nursing license required to dictate discharge summaries.
Experience:
Three years' experience in Health Information Management required. Ability to code diagnoses using DSM-V and
ICD-10-CM
coding manuals preferred. Knowledge /
Skills:
Knowledge of medical and psychiatric terminology. Knowledge of DSM-V and
ICD-10-CM
coding and ability to abstract charts. Must be highly proficient in computer applications and MS Products.
General Working Environment:
Hospital setting. Staff working in this position may be required to intervene and assist with aggressive patients. Works in a well lighted and ventilated setting, is subject to frequent interruptions, and is involved with the patients and personnel throughout designated unit under varying conditions and circumstances. Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to
HIV and Hepatitis B Viruses Physical Requirements :
Ability to communicate verbally and in writing. Must be able to lift a minimum of 50 lbs. and move frequently in completion of job requirements. Must be able to walk, stand, sit, and read for long periods and reach above and below the waist, turn, twist, and manipulate office equipment. Able to handle a variety of repetitive tasks and multiple assignments under deadlines with short notice. Able to participate in crisis prevention intervention. The Physical Requirements described herein are representative of those, which must be met by an employee to successfully perform the primary functions of this job.
