SUMMARY OF POSITION

Responsible for working with newly admitted patients to obtain and document an accurate medication list to help avoid medication duplications, omissions, and errors. The information obtained will be entered into the electronic health record to be used by physicians and other health care providers.

MINIMUM QUALIFICATION

(S): High School Diploma or equivalent required. Current Pharmacy Technician certification or currently licensed to practice as a licensed practical nurse by state of employment or holds a multistate LPN license through the interstate Nurse Licensure Compact.
