Job Overview We at Familia Health Clinic are seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team, dedicated to delivering exceptional patient care across diverse clinical settings. By lingual (English & Portuguese / Spanish) is desired. In this vital role, you will utilize your advanced nursing skills to assess, diagnose, and manage patient health, ensuring comprehensive treatment plans that promote wellness and recovery. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories across outpatient and inpatient settings. Develop and implement personalized care plans, incorporating evidence-based practices and current clinical guidelines. Administer medications, including injections and IV infusions, following proper protocols and safety standards. Perform specialized procedures such as suturing, catheterization, spinal taps, dermal fillers, and esthetic laser treatments with precision and care. Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks for accurate documentation of patient encounters, medical records, CPT coding, ICD-10/ICD-9 coding, and billing processes. Collaborate with multidisciplinary teams including physicians, nurses, therapists, and case managers to coordinate patient care effectively. Engage in telehealth consultations and participate in clinical research initiatives to advance healthcare practices. Manage acute pain cases, perform triage assessments in urgent care or emergency room settings, and oversee post-acute discharge planning for diverse patient populations including geriatrics, pediatrics, neonatal ventilator management, hospice & palliative medicine, and behavioral health. Qualifications Valid Nurse Practitioner license with current certification to practice independently. Proven experience in multiple clinical environments such as hospital medicine, emergency medicine (Level I & II trauma centers), ICU/PICU care, dialysis units or occupational health settings. Strong proficiency with EMR/EHR systems including Epic, Cerner or Athenahealth; experience with medical imaging and laboratory processing is a plus. Demonstrated expertise in procedures like suturing, phlebotomy, venipuncture, sterile processing techniques and airway management. Knowledge of pharmacology including psychopharmacology for behavioral health patients; familiarity with CPT coding and ICD coding standards essential for accurate documentation. Experience working with diverse populations such as assisted living residents, individuals with developmental disabilities or memory care needs; familiarity with Workers' Compensation law is advantageous. Critical care experience including ICU or neonatal ventilator management; trauma center background highly preferred. Additional skills include gastroscopy or gastric lavage procedures; infectious disease care; pain management strategies; utilization review; home care services; occupational health protocols; telehealth experience; and expertise in geriatrics or dementia care.
