Professional Growth Participate in:
Quality improvement initiatives Collaborative care planning Clinical protocol development Team-based healthcare innovation What You'll Do As an Advanced Practice Provider at GSCH, you will: Provide comprehensive primary care services across the lifespan Perform preventive health exams, diagnosis, treatment, and follow-up care Manage acute and chronic medical conditions Coordinate care and referrals with internal and external providers Collaborate closely with multidisciplinary healthcare teams Document patient care accurately within the EHR system Participate in provider meetings, quality initiatives, and clinical collaboration activities Deliver care with professionalism, compassion, and respect ✅
Minimum Qualifications APRN Candidates Must Have:
Current NH RN and APRN licensure Board Certification through ANCC and/or AANP Minimum of 3-5 years of experience practicing as a Nurse Practitioner R equired Current DEA registration Current BLS certification (ACLS/PALS preferred)
PA Candidates Must Have:
Current NH Physician Associate license NCCPA Certification Minimum of 3-5 years of experience practicing as a Physician Associate R equired Current DEA registration Current BLS certification (ACLS/PALS preferred) What We're Looking For We're seeking providers who are: Compassionate and patient-focused Team-oriented and collaborative Comfortable working in a fast-paced outpatient setting Passionate about community health and accessible care Adaptable, professional, and solution-oriented Experience in: Primary Care Community Health Family Medicine FQHC environments is highly valued, but mission alignment and strong patient care skills are equally important.
