Nurse Practitioner

Job Summary SoCal Nursing Wound Care, PC is seeking a compassionate and motivated Nurse Practitioner to provide advanced mobile wound care services to patients in home health, hospice, assisted living, and residential settings. This role focuses on delivering high-quality, evidence-based wound care directly to patients where they live while collaborating with home health agencies, hospice teams, caregivers, and physicians. The ideal candidate is confident working independently in the field, has strong clinical judgment, and is passionate about improving wound healing outcomes and patient quality of life. •pay range is based on experience and starting hourly rate will be up for negotiation after 6 months of hire. Responsibilities Perform comprehensive wound assessments and ongoing evaluations Develop and implement individualized wound treatment plans Perform procedures including sharp/mechanical debridement, Ultramist therapy, dressing applications, and advanced wound therapies Apply and manage skin substitutes and other advanced wound care modalities as appropriate Document visits accurately and thoroughly in the EMR (NextGen). Coordinate care with physicians, home health agencies, hospice teams, and caregivers Educate patients and families on wound prevention and treatment plans Monitor treatment progress and adjust plans of care accordingly Maintain compliance with Medicare and regulatory documentation standards Requirements Active California Nurse Practitioner license Board Certification as a Nurse Practitioner Strong assessment and procedural skills Ability to work independently and efficiently in a mobile setting Excellent communication and documentation skills Valid driver's license and reliable transportation Compensation & Benefits Competitive hourly or per-visit compensation Flexible scheduling Mileage reimbursement Clinical support and onboarding provided Opportunity for growth within a rapidly expanding mobile wound care practice Ideal Candidate We are looking for someone who is compassionate, detail-oriented, clinically strong, and comfortable building relationships with patients, families, and referral partners.
