Hospice and Palliative Care Nurse Practitioner Full-time, Monday through Friday, 8am-4:30pm About VNA Hospice NWI VNA Hospice of Northwest Indiana is the region's most trusted nonprofit hospice provider, serving families for over 50 years. Our mission is to deliver compassionate, patient-centered care that honors dignity, comfort, and quality of life. As part of our interdisciplinary team, you will help support patients and families during some of their most meaningful moments. Position Summary VNA Hospice NWI is seeking a dedicated Nurse Practitioner to provide high-quality palliative and hospice care across home settings, long-term care facilities, and our inpatient hospice center. This role is ideal for an NP who values relationship-based care, autonomy, and a supportive team environment. Key Responsibilities Perform comprehensive palliative and hospice assessments Manage symptoms including pain, dyspnea, anxiety, nausea, and agitation Collaborate closely with hospice physicians, RNs, social workers, chaplains, and volunteers Participate in interdisciplinary team meetings and care planning Provide education and emotional support to patients and families Assist with advance care planning and goals-of-care discussions Document all encounters in the EMR in accordance with regulatory standards Support transitions of care and coordinate with community partnersRequirements:
