Job Description

We are seeking an experienced APRN with nephrology expertise to deliver high-quality, patient-centered care in skilled nursing and long-term rehabilitation settings. This in-person role involves managing complex renal conditions and requires availability 3-5 days per week, including weekends and occasional after-hours on-call coverage. 1099 role
Responsibilities:
Deliver direct nephrology care in skilled nursing, rehabilitation, and long-term care settings Perform focused assessments and comprehensive physical examinations on patients with acute kidney injury and chronic kidney disease Evaluate, diagnose, and manage acute kidney injury and chronic kidney disease Develop and implement individualized treatment and monitoring plans Monitor patients' renal function, lab results, and vital signs; interpret and act on findings appropriately Complete timely and accurate documentation in the EMR/EHR Coordinate care with other specialties, attending physicians, nursing staff, and ancillary services Ensure continuity and quality care with prompt interventions to prevent decompensation Participate in interdisciplinary rounds and initiatives to reduce nephrology-related hospital readmissions Educate patients and families on renal conditions, medications, lifestyle changes, and warning signs Comply with infection control standards, regulatory requirements, and facility policies
Required Qualifications:
Active Florida APRN license Active NPI number Minimum of 1 year of nephrology or related specialty clinical experience Minimum of 3 years of nursing experience (skilled nursing or post-acute care preferred) Current BLS and ACLS certification Strong clinical assessment, communication, and independent decision-making skills Experience with EMR/EHR systems (PointClickCare and/or Matrix preferred) Ability to work independently in a fast-paced clinical environment Knowledge of post-acute care and geriatric best practices Working knowledge of state and federal healthcare regulations, HIPAA, and patient safety standards Excellent written and verbal communication skills for documentation and interdisciplinary collaboration Compassionate, patient-centered approach to care DEA registration (preferred
