Key Responsibilities:
Provide on-site wound care and patient rounding services across senior communities. Manage pressure injuries, ulcers, post-surgical wounds, and complex cases. Perform assessments, medication management, diagnostics, and follow-up care. Collaborate with physicians, PAs, nursing staff, and care teams. Support chronic disease management and reduce unnecessary hospital utilization.
Qualifications:
Active Minnesota Nurse Practitioner license in good standing. Wound care, geriatrics, post-acute, SNF, home health, or primary care experience preferred. Strong clinical judgment and ability to manage complex patient populations. Valid driver's license and reliable transportation required.
