NURSE PRACTITIONER

Hospice & Palliative Care of Iredell County is recruiting for a full-time Nurse Practitioner to provide care to patients with advanced life-threatening and life-limiting illnesses in Iredell County and surrounding areas. Candidates should be compassionate, self-directed, and comfortable providing care in a variety of settings including homecare, nursing facilities, assisted living facilities, and hospice inpatient units. This position will primarily work with the Hospice Interdisciplinary Teams to provide hospice and palliative care services in an outpatient setting under the direction of the Chief Medical Officer. Responsibilities include pain and symptom management, completion of hospice Face-to-Face visits, symptom management visits, and timely documentation in accordance with Medicare guidelines. Additional responsibilities include education and counseling for patients and families regarding disease progression and prognosis, completion of Advance Care Directives, and serving as an educational resource within the organization and the community. Hospice and palliative care experience strongly preferred.
