Key Responsibilities:

Provide advanced wound care and rounding services across senior communities. Manage pressure injuries, ulcers, post-surgical wounds, and complex cases. Support proactive care initiatives and reduce avoidable hospital utilization.

Qualifications:

Active Minnesota Nurse Practitioner license in good standing. Wound care, SNF, geriatrics, post-acute, primary care, or related experience preferred. Strong clinical judgment and comfort managing complex patient populations. Valid driver's license and reliable transportation required.
