Job Description

Overview Make a meaningful impact by helping patients avoid unnecessary hospitalizations while building long-term, trusted relationships. Physician's Services Group (PSG) is seeking a part-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our Post-Acute Care team in the Estill, SC area. This role offers schedule flexibility, clinical autonomy, and a collaborative care environment focused on improving outcomes for geriatric and post-acute patients within skilled nursing and long-term care settings. Schedule and Work Environment Part-Time | Flexible Weekday Schedule In-person care within Post-Acute and Skilled Nursing Facilities Work closely with attending physicians and interdisciplinary care teams Flexible structure with strong work-life balance Opportunity for long-term growth within PSG What You'll Do Provide comprehensive primary and post-acute care to patients in skilled nursing and assisted living settings Conduct patient assessments, diagnose conditions, and develop treatment plans Manage chronic conditions and support preventative care initiatives Collaborate with physicians, nursing staff, and facility teams to ensure continuity of care Document patient encounters accurately and in compliance with clinical standards Help reduce hospital readmissions through proactive, patient-centered care Build strong relationships with patients, families, and facility staff Compensation and Benefits Competitive compensation based on experience and schedule availability Performance-based incentives available Flexible scheduling Collaborative and supportive clinical environment Opportunity to expand into additional coverage days or nearby facilities if desired Qualifications Active Nurse Practitioner or Physician Assistant license in South Carolina, or ability to obtain Board Certified (AANP, ANCC, NCCPA, or equivalent) Strong clinical decision-making and documentation skills Experience in post-acute, skilled nursing, hospital, primary care, or outpatient settings preferred Interest in geriatric medicine, chronic disease management, and preventative care Ability to work independently while collaborating with a broader care team Why Join PSG?
