Job Description

Access Medical Clinic (AMC) is seeking clinicians to join our team that have the desire, drive, and dedication to assist AMC in providing great care to the communities we serve. The Clinic Advanced Practitioner (APP) is an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) who serves as primary provider in a Rural Health Clinic setting. In this role you will provide individualized care based on each patient's unique medical needs and lifestyle factors, including age, for optimal health and wellness in patients of all ages, including pediatric and seniors.
Details include:
Perform comprehensive patient assessments, including history, physical exams, and diagnostic evaluations. Diagnose and manage acute and chronic medical conditions. Develop, implement, and monitor individualized treatment plans. Prescribe medications and therapies in accordance with state and federal regulations. Order, interpret, and review laboratory tests, imaging, and other diagnostic studies. Provide preventive care, health education, and patient counseling. Document patient encounters accurately and timely in the electronic medical record (EMR). Collaborate with physicians, nurses, and other healthcare professionals to coordinate care. Refer patients to specialists and follow up on consults as appropriate. Ensure compliance with company policies, clinical protocols, and regulatory requirements. Participate in quality improvement initiatives and continuing education. Lead clinic staff in behaviors, actions and attitude (e.g. Nurses, Receptionists) in delivering excellent patient care. Provide guidance as necessary to ensure quality professional services and patient satisfaction. Maintain behavior that is compliant with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA. The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with customers, co-workers and managers. Perform other duties as assigned. Active, unencumbered state license. Nurse Practitioners must be permitted to treat all age groups, from infant to geriatric, as per their board specialty, such as Family Nurse Practitioner. DEA and BLS certification. DOT certification Strong interpersonal communication and knowledge of clinical practice standards.
