Nurse Practitioner Supervisor / Physician Assistant - Day Treatment Center (DTC)
Location:
Crown Heights, Brooklyn (Park Place) | Orientation in the
Bronx Schedule:
Monday-Friday | 8:30 AM - 5:30 PM (1-hour lunch)
Type:
Full-Time | Contract with potential for temp-to-perm We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to take on a Supervisor-level role within a Day Treatment Center (DTC) setting. This position is ideal for a clinically strong provider with leadership ability and experience in geriatrics, primary care, or internal medicine . Under the direction of the Chief Medical Officer, the NP/PA will oversee the clinical, administrative, and organizational aspects of patient care while ensuring high-quality outcomes and regulatory compliance. What You'll Do Provide comprehensive primary care services to adult and geriatric patients Perform assessments, evaluations, and ongoing follow-up care Manage chronic conditions and coordinate specialty services Supervise and support clinical staff as needed Collaborate with interdisciplinary teams (nursing, social work, rehab) Ensure accurate documentation in compliance with Medicare/Medicaid guidelines Participate in care planning, case reviews, and quality initiatives Support homecare services as needed Must-Have Qualifications Active New York State NP or PA license DEA license Medicare & Medicaid provider IDs Experience in Geriatrics, Primary Care, or Internal Medicine Strong leadership, clinical, and documentation skills Nice to Have Prior supervisory or lead experience Homecare experience or willingness to provide home-based care Experience in DTC, PACE, or managed care settings Additional Details Crown Heights (Park Place) location with Bronx orientation Stable weekday schedule - no nights or weekends Opportunity for long-term placement or permanent hire Competitive hourly compensation #INDNYALLIED
