This position is in salary group UHP-18; click here to review the current UHP Pay Plan UConn Health is looking for a highly skilled Palliative Care APRN to join the Oncology Supportive Care Service providing specialized supportive/palliative care to cancer patients. The candidate will be part of the interdisciplinary team collaborating with Oncology providers to co-manage patients during all phases of their disease process. This highly desirable day shift is located in the award winning Neag Comprehensive Cancer Center.
SCHEDULE
: Full time 40 hours per week - 5 eight hour shifts. Clinic hours are 8:00am - 4:30pm and minor holidays as needed. This crucial role is responsible for independently performing a full range of tasks in delivery of direct clinical care services involving medical therapeutics to high risk patients. This position will be in the Outpatient Pavilion Cancer Center in Farmington, CT.
MINIMUM QUALIFICATIONS, KNOWLEDGE, SKILLS, ABILITIES
Considerable knowledge of anatomy and physiology Knowledge of oncology, palliative care principles, and complex disease states Knowledge of medications, therapeutic interventions, and potential interactions/reactions Excellent counseling and patient education skills Exceptional interpersonal and collaborative communication skills Strong oral and written communication abilities Ability to work independently and collaboratively within an interdisciplinary team Ability to work effectively under pressure in a fast-paced clinical environment Ability to formulate comprehensive, patient-centered plans of care Strong clinical judgment and decision-making ability Strong physical assessment and clinical decision-making skills
EXAMPLES OF DUTIES
Assumes direct patient care and co-management responsibilities for a complex oncology patient population Obtains comprehensive medical histories, performs physical examinations, and evaluates patient conditions Develops and implements medical plans of care in collaboration with the primary oncology team Performs outpatient consultations and follow-up visits while working closely with the multidisciplinary team to ensure patients receive supportive services consistent with the plan of care Participates in multidisciplinary supportive care meetings, tumor boards, and disease-specific team meetings Cross-trains to provide coverage and support within the Cancer Survivorship Clinic to promote continuity of care and operational flexibility Works with leadership to ensure all Commission on Cancer (CoC) standards are achieved and maintained Acts as a liaison between supportive/palliative care, oncology providers, navigators, nursing staff, and ancillary services Intervenes appropriately in crisis situations or patient deterioration Provides routine, symptom-focused, and illness-related care Orders medications, diagnostic testing, and evaluates results Assumes responsibility for patient follow-up, coordination of care, and referrals to additional healthcare providers as appropriate Maintains accurate and timely patient medical records and documentation May participate in the development of policies and procedures Participates in quality improvement initiatives and system-of-care delivery changes May participate in clinical research initiatives Provides patient and family counseling and education Participates in medical student and healthcare professional education Acts as a clinical consultant and resource to peers and other healthcare professionals Performs all other duties as required

Certification as an Advanced Practice Nurse from an accredited Nurse Practitioner program Incumbents in this class must possess a valid license to practice in the State of Connecticut
PREFERRED EXPERIENCE
Two (2) or more years working in Oncology as an APRN Two (2) or more years working in Specialty Palliative Care in an inpatient and/or outpatient setting Demonstrated knowledge and expertise in Palliative & Hospice Care; ACHPN certification preferred or within the 1st year EPIC experience
FULL TIME MINIMUM EQUIVALENT SALARY
