Nurse (RN/LPN) - Night shift

Job Description Sochi Home Health is a Medicaid/ Medicare home health, ACHC accredited agency. We provide skilled and non-skilled services to clients in Montgomery and surrounding counties. We provide services for clients of all ages in the comfort of their homes. We are looking for dedicated RNs and LPNs with nursing experience, to join our team. New graduates are also welcome! The candidate will be dependable with the ability to communicate effectively with clients/patients and team members.

Responsibilities:

Provide one on one direct patient care as needed Work collaboratively with team members to help meet effective client outcomes Monitor patient's condition and notify appropriate personnel of any changes Communicate all pertinent clinical information to home care team members as indicated Provides patient & family teaching regarding disease process, prevention, infection control, home safety, & medications/possible side effects Maintain appropriate documentation as required by the agency Provide quality patient care Through development, implementation & evaluation of individual patient care plans. Follows established professional standards of care, Agency policy and procedures Perform any other patient care related job as assign.

Qualifications:

Graduate of an accredited school of nursing Current Ohio nursing license, in good standing One-year clinical experience in healthcare setting Must have current Basic Life Support (BLS) certification
