Occupational Therapist

~Occupational Therapist~ Heritage is seeking a consistent PRN or Part-Time Supervising OT for our Madison location. 1-2 days a week 5-10 hrs a week PRN or 10-15hrs a week Part-Time Join our in-house therapy team! Our skilled nursing facility located in Madison, Ohio provides an inviting, home-like environment conveniently located near I-90. This 120-bed facility has private rehabilitation suites with easy access to the therapy gym. The therapy staff and interdisciplinary team strive to create a comprehensive care plan that is unique to each short-term, outpatient, or long-term patient. Join our in house therapy team at Heritage Rehab as an Occupational Therapist dedicated to providing exceptional care. • Responsibilities:

• Conduct comprehensive assessments to develop personalized treatment plans for patients.
• Administer therapeutic activities and exercises to enhance patients' functional abilities.
• Educate patients and their families on adaptive techniques and equipment for daily living tasks.
• Collaborate with interdisciplinary teams to ensure holistic patient care.
• Maintain accurate documentation of patient progress and treatment plans.

• Qualifications:

• State licensure as an Occupational Therapist.
• Excellent communication skills and ability to work effectively in a fast-paced environment.
